Our Elder Home Care Services

Southern California Elder Home Care provides home care for a wide range of needs. Our caregivers are expertly trained to provide both general and specific types of care depending on the need of our clients. After evaluations, a caregiver is assigned who is capable of providing the type of care necessary.



Relocation Services

We are fully aware of how stressful it can be making the arrangements to leave the hospital or rehab center to return home. We can alleviate the burden by taking on all of those tasks.

Southern California Elder Home Care will:

  • Work with the patient and their family to determine the patient's post-release needs.
  • Work with the facility to ensure that equipment needs are identified.
  • Ensure that the equipment is requested through the proper Medicare or insurance coverage.
  • Arrange for the delivery of the required equipment to the patient's home.
  • Meet the patient and family on the day of discharge from the facility.
  • Assist in actual transfer from the facility to the home.

The only costs your have are the cost of the equipment and of other related charges.  Our Relocation Services are an integral part of our Caregiving Services and at no additional cost.

General Services:

  • Assisting with doctor’s appointments, medications, and exercises
  • Managing transportation
  • Providing physical, mental and emotional support
  • Assisting with daily activities such as personal care and hygiene

In addition, our plans can include one or more specific areas of care, including:

Care and Companionship Services:

  • Companionship
  • Shopping and paying bills
  • Meal preparation and eating assistance
  • Exercise and recreation
  • Tracking and monitoring changes in health
  • Light housekeeping
  • Transportation to/from doctor's appointments
  • Running errands
  • Doing laundry.

Dementia and Alzheimer's

People with dementia may have problems with short-term memory, keeping track of a purse or wallet, paying bills, planning and preparing meals, remembering appointments, or traveling out of the neighborhood.

Stroke Patients

Post-stroke recovery varies for each person. We work conscientiously with both patients and their families to ensure we meet their specific needs in the areas of

  • Planning out the stroke survivor’s care, including setting routines
  • Assessing the stroke survivor’s medical needs, communicating with healthcare professionals, and advocating (medically) for the stroke survivor.

Heart Patients

Southern California Elder Home Care works conscientiously with heart patients and their families in assessing ongoing medical needs, communicating with healthcare professionals, and advocating (medically) for the patient.    

Parkinson's Disease

Parkinson’s disease does not just affect the person living with it – it affects the entire family and sometimes, an extended community of friends and other loved ones. Patients with this complex and debilitating condition need individualized nursing care tailored to their needs to give optimal symptom control and improve quality of life.

Physical Rehabilitation

Physical rehabilitation can help people regain body functions they've lost due to medical conditions or injury. Rehabilitation can help with range of motion, movement and mobility, as well as thinking and reasoning. Our agency provides physical rehabilitation services in accordance with a doctor's advice and recommendation.

Broken Bones

Hip and other fractures are serious injuries, particularly if you're older; complications can be life-threatening. A hip fracture almost always requires surgical repair or replacement, followed by months of physical therapy. California Elder Home Care provides tailored care plans that include help specifically designed to prevent additional injury and provide a safe environments for recuperation.
